2023

I remember telling myself at the start of the year that I'd leave my current workplace by 2022 December.

Whelp, it's 2023 and I'm still here.

Idk man.

Maybe I have too much insecurities.

Maybe I am lazy to have to adapt to new things.

Maybe it's because my parents keep telling me to leave.

Maybe it's all of that.

I kinda vaguely knew this would happen before stepping foot into society. I never was a person to leave anything halfway through. Unless I was kinda half dead. Like when I spiked a high fever in the middle of the day in school (though I already went to school with a fever).
Otherwise, I cannot name a single time which I proactively left or quitted something.

It's me I guess.

Should I just forget about leaving?

jumping into conclusions

Went to Sushi-GO with mum today
Ordered a cup of hot tea and a can of Qoo white grape drink.
The young waiter came and placed the Qoo Infront of me, the tea in front of my mum.
Hahaha.
Little did he know, I was the one who wanted the hot tea.
Hahahahaha.
Pocketed the coin token that were supposed to be used to try to claw an ice-cream from their ice-cream machine as a token of momento. Feel like pocketing the coins is more worth it.

Monday afternoons

Just another Monday afternoon shift in the ED...

Just as if it wasn't bad enough, we've got annoying patients who are annoyed at us just because they needed to wait.
One pt went: I have been here for 8 hours, where is my ward and bed?!?? (On repeat)

So, I went and explained, the hospital is very full, there are no beds available for the discipline you're admitted under, you'll need to wait for a bit longer. Beds aren't given so quickly especially on Mondays where there are lots of patients. You're lucky if you did not need to wait for more than 12hours.

Inner OS: Gosh. 8 hours only... and you're on a trolley bed...wait till you have to wait for 2 days (yes, this has happened to my patients when the bed situation was really bad)

So I went off to serve meds for my other patients... He had some bleeding going on in his GI tract, I had earlier aspirated blood from his nasogastric tube.

This patient continues loudly: What kind of hospital is this? I didn't want to come here in the first place. Don't even have food.

Mark my words, he had just munched on some bread just an hour ago. And it was 7pm, still within the time which we can serve dinner.

So, while diluting my medication with one hand, I went to another zone to get him some rice. I layed the bento box down on his table, and had not have a chance to speak when...

He commented sarcastically: Nothing to drink ah?

So I took a deep breath, and asked what drink would he like?

He continues with his comments about how lousy a hospital we were, ignoring my question.

So, I took a deep breath and repeated. "So what drink would you like?"

He said water, and I gave him some warm water. Chinese older patients usually like their drinks warm.

Thinking he would finally be appeased or at least for that moment, I continued to serve my meds for my sick patient.

Until I heard him comment loud for me to hear.

"So late then give food, I no appetite already!"

My inner OS: Well, then you can leave it. I don't care anymore. Deep breaths... Inner peace... Focus on my medications...

So I kinda blocked him out of my head afterwards.

Later he shouted again that he hasn't been given any medication.

Which was not true. We have given him his meds in the afternoon via IV route. His medication due at night was out of stock in our medicine cupboard so I had requested for it from the pharmacy. Which means = he had to wait.

And yep, he continues to complain.

Whatever. If you can be so energised and be complaining about everything, you're well enough not to need my attention for the time being.

I'm going to let him be while I will wait for his medication to be fetched by the porter.

But in the end I still took the initiative to run to the pharmacy (which wasn't near at all) to get his meds.

And well, after the medication he continues to complain about the bed waiting time.

I give up.

I'm finishing my shift.

I'm outta here.

I'm not going to let him get to me.

Let him be.

While all this was happening, another of my patient who came in with a fall and head injury was demanding to be discharged right after the CT brain scan was done for her, refusing to wait for the report.

Like... the heck?

She was like: I am fine! I can run! You don't need to hold on to me!
As I was WALKING her to the toilet. Oh and did I mention? She insisted that I attended to her right away even though I was obviously speaking to another patient's next of kin who was concerned that she is no longer allowed to stay in the ED with the patient ( had allowed her in for a while to let the anxious patient settle down ) and I was reassuring her and getting her contact information.

Oh god. 

After getting her back on the bed I don't know how she got down again after a while and started pestering the doctor at her desk. 

Goodness. You're not the only patient ok? And our doctor isn't the one writing the scan report. It's the radiologist who are trained to read and pick up things from the hundred of images they have to screen through for one patient.

I really didn't have the time to spare for all this nonsense. 
If you aren't prepared to wait, please do not enter the hospital premises. Hire your own private Doctor. End of story.

In the middle of all this another patient who just came in not long ago, has not gone for his Xrays, just had his bloods taken, was asking for food.

Like excuse me? Do you think you're at a hawker centre? Can't you eat before come to the hospital? Well guess what, you can't eat now since we have to make sure you're well and will not need any surgery or procedures soon to save you.
Sorry, but what can I do?

 Another patient: The doctor just told me that I can eat, can I order food from you?

Excuse me, do I look like a waitress?

But I still patiently explained, no sorry, we don't take orders for food in the ED, we only have porridge, bread and crackers, are you alright that I get you some porridge?

She looked taken aback, almost as if I told her watermelons grew from trees.

She went on saying no... I have gastric issues I cannot just eat porridge like that. 

I was like... Isn't porridge almost the best choice of food if you have problems with your stomach? If not porridge then what? Sashimi? I don't know you madame.

So I kindly explained to her, you can always get your family to bring you food and drop it off at the ambulance bay counter where our staff will bring it in to you.

So ... she couldn't say anything else. There's really nothing much we can do anyways. Next time, bring your own private chef.

nurse-cum-techsupport

The first half of my shift today: 
1. Cracking one of my patients' phone password for her and drawing the pattern on a piece of paper to remind her + writing her sons' number down cuz she keeps rmb-ing it wrong. 
2. Writing down and drawing step by step how another of my patient can WhatsApp call his daughter who is currently overseas.
Thank God it wasn't that busy then, both of them were starting to become anxious and irritable as they don't know how to contact their loved ones.
Am I a nurse-cum-techsupport now? HAHAHA 😂. But it felt really good and satisfying when I see them slowly calm down as they listen to me teach them and can tell me again how to do it while looking at the piece of paper.
The same feeling when my patients newly put on IDC can do a verbal return regarding the care of their IDC. :))

received this snapshot in my job chat group.
Wah.
If you just look at the numbers like that, it almost seem like we're doing quite well as compared to most other hospitals.
BUT
What about nurse to patient ratios?
No one bothered to look at that number.
Bad mistake.
Really bad.
So what if you have 1000 patients. If you have 250 nurses, one nurse can care for 4 patients. Perfect.
So what if you have 500 patients. If you only got 50 nurses, one nurse takes care of 10 patients. You will not be able to remember what the heck patient had before that relates to his or her current condition. You won't have the brain capacity to care whether the patient has been going hungry unnecessarily and that you may need to go fight the doctor that ordered NBM like they've gone zen and no longer need food.
You won't have time to care for your patients with patience and love because you're fighting for time to do basic care for the next one lying beside. And what about that one that is yelling at the back because he or she got so much needs and wants that we have no time to meet and cater to all of them?
And ten is almost like the average number of patients per Nurse these days... NETT. Not total per shift.
You see the difference?
We don't care how many patients there are, actually. 
We care about how many patients we each have to care for.
Period.

feeling a little out of place

Was scrolling through Facebook and curiously clicked on the profile pages of my colleagues.

Needless to say, all the Doctors came from reputable schools. Duh. They're the ones with straight As.
Then I realised a lot of the seniors also came from reputable school too. Hwa Chong, Raffles, NJC, River Valley, Nanyang...
I looked back at my profile.
Wow
Ok.

Growing up I always had this sense of pride when I compared myself to people and they are from neighborhood schools. 
As if I am from a different level.
Lol.
Such a bitch.
And those from RI and HCI probably think the same of people like me.

Idk man. There probably are people that don't bother maybe.

But idk why now I still have this sense of... inferiority almost.
Not very good for my mental health huh. But I can't seem to help it.
Can't help but wonder if it's part of the reason why I don't function as well as my colleagues.
Why I am not as outspoken and confident.
But looking back, I have got school mates that are outspoken and confident...
So i guess the problem just lies with me.
Hah.

Ystd was so busy with my shift I didn't get to go for break.
Today I took MC
Cuz of this weird pulling pain down my shoulder to the 4th and 5th fingers both sides. Took some Nsaids and Vitamin B. Hope it helps.

Now in a 70+ year old elderly. 
But he's got good veins. 
But maybe cuz I had Dr KJ behind me so I was kinda more relaxed.
Erm.
Anyways.
It counts 🤣

Paranoid Personality Disorder

So... I am starting to feel pretty sure my mom has this 

So, according to this web, sign and symptoms:
  • Doubt the commitment, loyalty, or trustworthiness of others, believing others are using or deceiving them.
## believes there are scammers on Youtube that pops a scam when she is watching videos, asking her for money.

  • Are reluctant to confide in others or reveal personal information due to a fear that the information will be used against them
## refuses to put profile pictures (not even of random Google photos she likes) or birthday on her online accounts, refuses to put even her name initially. Thinks ppl can make use of that and do bad to her.

Recently she's saying that my aunt (my father's Sister) has casted black magic on her using her birthday and that's what has caused her hospitalisation few years back.

To this I argue: you means every one that ends up hospitalised are there because of black magic? Who is it so free huh. Every day I seen hundreds of patients getting admitted. Who isit casting spells on them? Got so many ppl nothing better to do casting spells everyday ah? Black magic work so well why not do it to benefit themselves instead.

Then she smiles and shut up.

  • Are unforgiving and hold grudges
Always holding grudges. Lost count. Example, repeatedly saying that if it was not for that worker that did not place the metal rod properly and causing her to hit her head and then all her studies went downhill and she became a drop out eventually and that's why she married my dad cuz of her low education level.

  • Are hypersensitive and take criticism poorly
My dad has gotten the hard end for this one. 

  • Read hidden meanings in the innocent remarks or casual looks of others
Recently was on the public transport back home with her after getting groceries from Jurong point.
Came home and she was talking about the group of ang mohs in front of us giving us glances and talking in a foreign language. I noticed that they did glance once or twice but didn't think much about it. Like whatever. 
She feels that they are talking bad about us.
I asked her if she had understood their language.
She said no but still insisted on her gut feeling.
I told her maybe they just think we're hot chicks. 
She laughed.

  • Perceive attacks on their character that are not apparent to others; they generally react with anger and are quick to retaliate
Always. 

  • Have recurrent suspicions, without reason, that their spouses or lovers are being unfaithful
Uhm this she never reveal if she does think so.

  • Are generally cold and distant in their relationships with others, and might become controlling and jealous
Very cold and distant towards her mother and siblings. Though the long distant relationship may have been the cause.

  • Cannot see their role in problems or conflicts and believe they are always right
Stubborn in her beliefs 

  • Have difficulty relaxing
Very anxious. Especially when out of the house. Becomes very reliant on us. Gets jittery sometimes.

  • Are hostile, stubborn, and argumentative
Stubborn. Bold. Caps. Italic. Underline. Though she don't usually win me in an argument. Guess it runs in the genes.

...whut

Dreamt a weird dream.

Somehow was unwell at work, fainted or smth. 
Woke up and the Dr was telling me that I've got this gyne issue 
"Neemoid ecchymosis"
Lol this condition does not exist.
But in the dream it's something along the line where I had this weird swelling and bruise on my womb.
So I had to undergo surgery for it immediately.
Was calling my mum about it right before the op with the OT staff's phone. It almost felt like i was ready to die.
Very quickly the op finished.
Saw the many specimens from others hanging around.
Then I was discharged.
Lol.
Was on my way home calling my sis to find my mom, only to know she's told my dad and he's bringing my paternal side of the family to the hospital to visit me.
Lol.

Then.
I peed.
In real life too.

Taiwan still has it's leisure travel boarders closed though.

So I kinda silently made up my mind that if I do go Taiwan again,

I want to book hedo hotel again.

For it's interior and it's service.

my advanced care planning

So I was whatsapping a colleague who just left ED to Carehub and somehow the conversation led to staying single ... 
So my plan is to earn as much as I can then when I finally have enough of everything and have reached the state where I can no longer care for myself/ have enough money to sustain crazy healthcare expenses... I'll hire a private nurse to take me overseas, and then start my countdown to my last days. Go sightseeing for a while (if I still can see) then admit to a hospital for euthanasia.
What's the point of being alive, have got no family around anymore (assuming my parents have passed when I reached that state), and have no quality of life.
Right?
Hmm... Looks like I've got quite a bit of a choice huh. Hahaha
